The De Mare composer credit is missing from some pressings of the single as well as the sheet music...

Quite possibly, though, it was a pseudonym for Tony Meehan (musical director and producer).

As well as Applejack in 1963 for the Jet / Tony partnership, there was also Some People for The Eagles, Carol Deene and Jet in 1962.

Don't know if this is the same Johhny Worth but I'm sure I remember
a Johnny Worth making recordings for Woolworth's Embassy label.

The same person.

Embassy, in particular during the 50s/early 60s (as a budget covers label), was very much a home for well-known artists using pseudonyms, a practice that dates back to the earliest days of the recording industry.
